Kansas City, Kan- Ali Baker, born with a congenital heart defect, transferred from pediatric to adult care at the University of Kansas Health System. 
The seamless transfer was facilitated by the blended system between Children's Mercy and our health system that starts when patients reach 14-16 years old.
